Service Description
July 9-12 (9am to 12 pm) AGES 8 - 12 YEARS Camp includes: Growth Mindset activities, Team Building/Social Skills games, STEM/Wind technology, Body Flight instruction, 3-4 flights per day in wind tunnel, IBA Membership (International Body Flight Association), Official IBA Sign-offs on skills performed, fly with team mates (depending on progression), Certificate of completion. After registering you will receive an invoice by email for payment. The total amount of $825 is due in order to reserve your spot. If you do not receive your email from Rhizome Learning Solutions after registering, please check your spam or junk folder. Max participants for this event is 12 students. The Minimum participants for this event is 8 students. If the minimum number of students do not register, the event will be cancelled. Attendees will be notified within 48 hours of the event and a refund for the full amount will be issued.
Cancellation Policy
Reservation Policy To reserve your child’s assessment appointment, a non-refundable deposit of $150 is required at the time of booking. The remaining balance of [$1,000] is due on the day of the assessment or before. Rescheduling requests made 48 hours or more in advance will carry the deposit forward to the new appointment. Payment options include credit/debit, Zelle, or approved alternatives. Cancellation Policy All appointments must be canceled or rescheduled at least 48 hours in advance. Cancellations made less than 48 hours before the appointment will result in forfeiture of the deposit. No-shows or same-day cancellations are billed at 25% the evaluation rate. Consent for Services I understand the nature and purpose of this psychoeducational assessment and give permission for Jacqueline Choudhry, M.ED., Ed.S., NCED, of Rhizome Learning Solutions, LLC, to complete the evaluation as described. This assessment is not intended to diagnose medical conditions but may provide documentation for educational planning, including support for education therapy, accommodations, or services under Section 504 or IDEA. Results and recommendations will be shared in a written report and during a scheduled parent feedback meeting.
