Includes all components in Option 1, with two additional tools to provide a broader view of functioning. Additional Assessments: Vineland-3 Adaptive Behavior Assessment – $150 Evaluates everyday life skills in communication, socialization, and daily living. This helps us understand how your child navigates the world outside of academic settings and provides insight into independent functioning and goal setting. BRIEF-2 Executive Functioning Assessment – $150 A parent- and teacher-completed rating scale that evaluates your child's ability to plan, organize, regulate emotions, and manage impulses. This tool is particularly helpful in identifying executive functioning challenges often seen in students with ADHD and ASD and creating targeted interventions.
Cancellation Policy
Reservation Policy To reserve your child’s assessment appointment, a non-refundable deposit of $50 is required at the time of booking. The remaining balance of [$1,000] is due on the day of the assessment. Rescheduling requests made 48 hours or more in advance will carry the deposit forward to the new appointment. Payment options include credit/debit, Zelle, or approved alternatives. Cancellation Policy All appointments must be canceled or rescheduled at least 24 hours in advance. Cancellations made less than 24 hours before the appointment will result in forfeiture of the deposit. No-shows or same-day cancellations are billed at 25% the evaluation rate. Consent for Services I understand the nature and purpose of this educational assessment and give permission for Jacqueline Choudhry, M.ED., Ed.S., NCED, of Rhizome Learning Solutions, LLC, to complete the evaluation as described. This assessment is not intended to diagnose medical conditions but may provide documentation for educational planning. Results and recommendations will be shared in a written report.
